Forms for girls

All forms are available in two different PDF formats. The interactive PDF forms are best if you are wanting to fill them out electronically and then email them. The standard PDF forms are a smaller file size and are the best for printing and filling them in by hand. Either form is acceptable.

  • Girl Permission Form (GP)
    For permission for a girl to run a girl led camp, outdoor recreation or adventure activity.

  • Report Form (R)
    Collect detailed and accurate information about the camp, outdoor recreation or adventure activity for GirlGuiding NZ reporting purposes.

  • Consent Form (C)
    Parent/caregiver consent and up to date medical information for any camp, outdoor recreation or adventure activity occurring outside the normal unit meeting time or place.

  • Intentions Form (I)
    For groups who are taking part in camps, outdoor recreation or adventure activities. See Safety Guidelines for Programme Activities to determine whether or not this form is required for you event.

    For girl or adult leaders who are organising/running camps, outdoor recreation or adventure activities to identify and manage hazards.

  • ACT1a Workshop Application Form

    Rangers need to use the ACT1a form to attend the Queen’s Guide Residential Course or if they want to attend a Zone Workshop as part of their Practical Leadership Certificate (an optional requirement for the Queen’s Guide Award).
